The invention relates to a piston machine (10, 40) which comprises at least a cylinder (12a, 12b) and a cylinder head (18a, 18b) closing an opening of the cylinder (12a, 12b). Further, the piston machine (10, 40) has a piston (22a, 22b) arranged, at least in part, within the cylinder (12a, 12b) and arranged movably in its longitudinal direction with the aid of a connecting rod (28a, 28b). The piston (22a, 22b) has an opening extending in its longitudinal direction, within which at least a partial area of a guiding element (30a, 30b) firmly connected to the cylinder head (18a, 18b) is arranged.
