A system and method for generating statements for use by a user in collecting clinically relevant information about a patient for a selected examination type. The system comprises a storage including a plurality of statements having statement concepts assigned to each of the plurality of statements, the statement concepts being descriptors for defining each of the plurality of statements. The system comprises a receipt module configured for receiving a request having at least one parameter including an exam concept associated with the selected exam type for defining a characteristic of the selected exam type. The system comprises a matching module configured for comparing the exam concept to the statement concepts assigned to each of the plurality of statements, the comparison for selecting matching statements from the plurality of statements having the exam concept, the comparison resulting in a statement set containing the matched statements. The system comprises a generation module configured for generating a statement form including the statement set, the statement form for use by the user in collecting the clinically relevant information about the patient.
